Gastroesophageal reflux disease (GERD) is a condition in which the esophagus becomes irritated or inflamed because of acid backing up from the stomach. The esophagus or food pipe is the tube stretching from the throat to the stomach. When food is swallowed, it travels down the esophagus.


 The stomach produces hydrochloric acid after a meal to aid in the digestion of food. The inner lining of the stomach resists corrosion by this acid. The cells that line the stomach secrete large amounts of protective mucus. The lining of the esophagus does not share these resistant features and stomach acid can damage it.


Approximately 14 million Americans experience symptoms of GERD, including heartburn andesophageal inflammation, on a daily basis. Most manage their disease with medications such as H2 blockers and proton pump inhibitors (PPIs). Since pharmacological therapy does not treat the underlying root causes of reflux, the deteriorated anatomy of the antireflux barrier, life-long medication therapy is required.


Transoral Incisionless Fundoplication is a minimally invasive surgery that represents the latest advancement in the surgical treatment of GERD. Transoral Incisionless Fundoplication, using the  EsophyX™ device , has been performed in Europe for over four years. In October 2007, the EsophyX™ device received approval from the U.S. Food and Drug Administration, thus allowing the TIF procedure to be performed in the United States.


The EsophyX™ Transoral Incisionless Fundoplication (TIF) procedure is a novel surgical procedure for the treatment of reflux disease. The TIF procedure is based on the principles of surgical repair of the antireflux barrier and is intended to deliver similar benefits as the time-proven laparoscopic fundoplication, sometimes referred to as a Nissen Fundoplication.


The TIF procedure reconstructs a robust antireflux barrier, thus restoring the gastroesophageal junction back to what is believed to be its natural anatomical geometry.


The TIF procedure is a natural progression for Drs. Turgeon and Trad who have had a long-standing interest in the surgical treatment of GERD. Dr. Turgeon and Dr. Trad were the first surgeons in the metropolitan Washington area to perform the laparoscopic Nissen Fundoplication, in the 1990s, as a then minimally invasive treatment for the relief of reflux.


Dr. Turgeon and Dr. Trad traveled to Europe in March 2008 to receive training in Transoral Incisionless Fundoplication surgery and then performed surgeries in the United States under the supervision of their European instructors. They have been successfully performing the TIF procedure since May 2008.
